This variant lies in the VCP gene (transcript NM_007126.5) at coding-DNA position 266, where G is replaced by A; at the protein level this means replaces arginine at residue 89 with glutamine — a missense variant. Submitter rationale: This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 89 of the VCP protein (p.Arg89Gln). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of VCP-related conditions (PMID: 31866807; Invitae). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 2051539). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt VCP protein function with a positive predictive value of 95%. This variant disrupts the p.Arg89 amino acid residue in VCP. Other variant(s) that disrupt this residue have been observed in individuals with VCP-related conditions (PMID: 30955949), which suggests that this may be a clinically significant amino acid residue. For these reasons, this variant has been classified as Pathogenic.
