Uncertain significance — the classification assigned by GeneDx to NM_000742.4(CHRNA2):c.212A>G (p.Asn71Ser), citing GeneDx Variant Classification (06012015): p.Asn71Ser (AAC>AGC): c.212 A>G in exon 3 of the CHRNA2 gene (NM_000742.3). A variant of unknown significance has been identified in the CHRNA2 gene. The N71S variant has not been published as a mutation, nor has it been reported as a benign polymorphism to our knowledge. It was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The N71S variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties. This amino acid substitution does not occur within the transmembrane region of the protein, where the vast majority of pathogenic missense CHRNA2 mutations have been identified in association with epilepsy (Steinlein et al., 2010). However, this substitution occurs at a position that is highly conserved across species and in silico analysis predicts this variant is probably damaging to the protein structure/function. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic mutation or a rare benign variant. The variant is found in EPILEPSY panel(s).
