Uncertain significance for Autosomal recessive limb-girdle muscular dystrophy type 2Q; Epidermolysis bullosa simplex, Ogna type; Epidermolysis bullosa simplex with nail dystrophy; Epidermolysis bullosa simplex 5C, with pyloric atresia; Epidermolysis bullosa simplex 5B, with muscular dystrophy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_201384.3(PLEC):c.8870A>G (p.Lys2957Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the PLEC gene (transcript NM_201384.3) at coding-DNA position 8870, where A is replaced by G; at the protein level this means replaces lysine at residue 2957 with arginine — a missense variant. Submitter rationale: This sequence change replaces lysine, which is basic and polar, with arginine, which is basic and polar, at codon 2984 of the PLEC protein (p.Lys2984Arg). This variant is present in population databases (rs782462913, gnomAD 0.003%). This variant has not been reported in the literature in individuals affected with PLEC-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The arginine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
