Uncertain significance for Familial thoracic aortic aneurysm and aortic dissection — the classification assigned by Ambry Genetics to NM_017617.5(NOTCH1):c.383G>T (p.Arg128Leu), citing Ambry Variant Classification Scheme 2023: The p.R128L variant (also known as c.383G>T), located in coding exon 3 of the NOTCH1 gene, results from a G to T substitution at nucleotide position 383. The arginine at codon 128 is replaced by leucine, an amino acid with dissimilar properties. This variant was reported in a bicuspid aortic valve cohort (Debiec RM et al. Heart, 2022 Jun;108:1114-1120). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear.
