Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_006005.3(WFS1):c.784del (p.Ser262fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the WFS1 gene (transcript NM_006005.3) at coding-DNA position 784, deleting one base; at the protein level this means shifts the reading frame starting at serine residue 262, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the WFS1 protein in which other variant(s) (p.Pro885Leu) have been determined to be pathogenic (PMID: 10521293, 16806192, 28432734). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. This variant has not been reported in the literature in individuals affected with WFS1-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Ser262Alafs*25) in the WFS1 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 629 amino acid(s) of the WFS1 protein.
