Pathogenic for Primary ciliary dyskinesia — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_012144.4(DNAI1):c.-1_4del (p.Met1fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the DNAI1 gene (transcript NM_012144.4) at 1 bases upstream of the translation start (5' untranslated region) through coding-DNA position 4, deleting this region; at the protein level this means shifts the reading frame starting at methionine residue 1, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the DNAI1 protein in which other variant(s) (p.Arg124Cys) have been determined to be pathogenic (Invitae). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. This variant has not been reported in the literature in individuals affected with DNAI1-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change affects the initiator methionine of the DNAI1 mRNA. The next in-frame methionine is located at codon 178.
