NM_001267550.2(TTN):c.43748-1G>A was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the TTN gene (transcript NM_001267550.2) at the canonical splice acceptor site of the intron immediately before coding-DNA position 43748, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: c.38825-1 G>A: IVS186-1 G>A in intron 186 of the TTN gene (NM_001256850.1). Although the c.38825-1 G>A variant has not been reported as a disease-causing mutation or as a benign polymorphism to our knowledge, this variant destroys the canonical splice donor site in intron 186 and is predicted to cause abnormal gene splicing. The variant is predicted to lead to either an abnormal message that is subject to nonsense-mediated mRNA decay, or to an abnormal protein product if the message is used for protein translation. In addition, c.38825-1 G>A was not observed in approximately 6,000 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. However, c.38825-1 G>A is not located in the A-band region of titin, where the majority of truncating mutations associated with DCM have been reported (Herman D et al., 2012). Other truncating variants have been reported in approximately 3% of control alleles (Herman D et al., 2012).Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic mutation or a rare benign variant.
