This variant lies in the CAPN3 gene (transcript NM_000070.3) at the canonical splice donor site of the intron immediately after coding-DNA position 801, where T is replaced by G; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: This sequence change affects a donor splice site in intron 5 of the CAPN3 gene. RNA analysis indicates that disruption of this splice site induces altered splicing and may result in an absent or disrupted protein product. This variant is not present in population databases (gnomAD no frequency). For these reasons, this variant has been classified as Pathogenic. Disruption of this splice site has been observed in individual(s) with autosomal recessive limb-girdle muscular dystrophy (PMID: 9452114). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. Studies have shown that disruption of this splice site results in skipping of exon 5 and introduces a premature termination codon (PMID: 9452114). The resulting mRNA is expected to undergo nonsense-mediated decay.
