NM_000051.4(ATM):c.8584+2T>A was classified as Pathogenic for Ataxia-telangiectasia syndrome by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the ATM gene (transcript NM_000051.4) at the canonical splice donor site of the intron immediately after coding-DNA position 8584, where T is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: This sequence change affects a donor splice site in intron 58 of the ATM gene. RNA analysis indicates that disruption of this splice site induces altered splicing and may result in an absent or disrupted protein product. This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individual(s) with clinical features of ataxia-telangiectasia and/or colorectal cancer (PMID: 28170084, 28195393; Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. Studies have shown that disruption of this splice site results in activation of a cryptic splice site and introduces a premature termination codon (PMID: 28170084). The resulting mRNA is expected to undergo nonsense-mediated decay. For these reasons, this variant has been classified as Pathogenic.
