Likely pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004525.3(LRP2):c.8932T>C (p.Cys2978Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the LRP2 gene (transcript NM_004525.3) at coding-DNA position 8932, where T is replaced by C; at the protein level this means replaces cysteine at residue 2978 with arginine — a missense variant. Submitter rationale: This sequence change replaces cysteine, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 2978 of the LRP2 protein (p.Cys2978Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of Donnai-Barrow syndrome and/or LRP2-related conditions (internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 2008664). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt LRP2 protein function with a positive predictive value of 95%. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
