Likely pathogenic for Neutropenia, severe congenital, 1, autosomal dominant; Cyclical neutropenia — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001972.4(ELANE):c.581_597dup (p.Gly200fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the ELANE gene (transcript NM_001972.4) at coding-DNA position 581 through coding-DNA position 597, duplicating 17 bases; at the protein level this means shifts the reading frame starting at glycine residue 200, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change creates a premature translational stop signal (p.Gly200Argfs*18) in the ELANE gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 68 amino acid(s) of the ELANE protein. This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with neutropenia (internal data). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 2004561). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
