Likely pathogenic — the classification assigned by GeneDx to NM_000138.5(FBN1):c.6499A>C (p.Thr2167Pro), citing GeneDx Variant Classification (06012015): p.Thr2167Pro (ACT>CCT): c.6499 A>C in exon 54 of the FBN1 gene (NM_000138.4) A T2167P variant that is likely pathogenic was identified in the FBN1 gene. It has not been published as a mutation, nor has it been reported as a benign polymorphism to our knowledge. The T2167P variant was not observed with any significant frequency in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The T2167P variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. In silico analysis predicts this variant is probably damaging to the protein structure/function. Furthermore, missense mutations in nearby residues (A2160P, D2166N, D2168G, E2169G) have been reportede in association with Marfan syndrome and aortic aneurysm, supporting the functional importance of this region of the protein. Therefore, this variant is a strong candidate for a pathogenic mutation, however the possibility that it is a benign variant cannot be excluded. The variant is found in TAAD panel(s).
