Pathogenic for Landau-Kleffner syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001134407.3(GRIN2A):c.3609del (p.Ser1204fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GRIN2A gene (transcript NM_001134407.3) at coding-DNA position 3609, deleting one base; at the protein level this means shifts the reading frame starting at serine residue 1204, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the GRIN2A protein in which other variant(s) (p.Trp1271*) have been determined to be pathogenic (PMID: 29961510). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. This variant has not been reported in the literature in individuals affected with GRIN2A-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Ser1204Alafs*27) in the GRIN2A gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 261 amino acid(s) of the GRIN2A protein.
