Uncertain significance for Intellectual disability, autosomal recessive 42 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_024989.4(PGAP1):c.251C>T (p.Thr84Met), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the PGAP1 gene (transcript NM_024989.4) at coding-DNA position 251, where C is replaced by T; at the protein level this means replaces threonine at residue 84 with methionine — a missense variant. Submitter rationale: This sequence change replaces threonine, which is neutral and polar, with methionine, which is neutral and non-polar, at codon 84 of the PGAP1 protein (p.Thr84Met). This variant is present in population databases (rs114297432, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with PGAP1-related conditions. ClinVar contains an entry for this variant (Variation ID: 1991599). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on PGAP1 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
