Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004646.4(NPHS1):c.2053G>T (p.Gly685Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glycine with cysteine at codon 685 of the NPHS1 protein (p.Gly685Cys). The glycine residue is highly conserved and there is a large physicochemical difference between glycine and cysteine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals affected with NPHS1-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt NPHS1 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
