Uncertain significance for Dyskeratosis congenita, autosomal recessive 6; Pulmonary fibrosis and/or bone marrow failure, Telomere-related, 4 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002582.4(PARN):c.1178T>C (p.Met393Thr), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the PARN gene (transcript NM_002582.4) at coding-DNA position 1178, where T is replaced by C; at the protein level this means replaces methionine at residue 393 with threonine — a missense variant. Submitter rationale: This sequence change replaces methionine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 393 of the PARN protein (p.Met393Thr). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. This variant has not been reported in the literature in individuals affected with PARN-related conditions. This variant is not present in population databases (gnomAD no frequency).
