Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_207111.4(RNF216):c.1934G>A (p.Arg645Gln), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the RNF216 gene (transcript NM_207111.4) at coding-DNA position 1934, where G is replaced by A; at the protein level this means replaces arginine at residue 645 with glutamine — a missense variant. Submitter rationale: This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 645 of the RNF216 protein (p.Arg645Gln). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with cerebellar ataxia and hypogonadotropic hypogonadism (Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 1929393). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
