Uncertain significance — the classification assigned by GeneDx to NM_004415.4(DSP):c.1630A>C (p.Met544Leu), citing GeneDx Variant Classification (06012015). This variant lies in the DSP gene (transcript NM_004415.4) at coding-DNA position 1630, where A is replaced by C; at the protein level this means replaces methionine at residue 544 with leucine — a missense variant. Submitter rationale: A variant of uncertain significance has been identified in the DSP gene. The M544L variant has been observed in at least one individual from a cohort of individuals not selected for cardiomyopathy, arrhythmia, or family history of sudden cardiac death, who underwent exome sequencing (Ng et al., 2013); however, a follow-up cardiac evaluation was not reported. The M544L variant is not observed at a significant frequency in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The M544L variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties. This substitution occurs at a position where amino acids with similar properties to methionine (M) are tolerated across species. Nevertheless, in silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function.
