This variant lies in the ANK2 gene (transcript NM_001148.6) at coding-DNA position 10693, where G is replaced by A; at the protein level this means replaces glutamic acid at residue 3565 with lysine — a missense variant. Submitter rationale: p.Glu3565Lys (GAG>AAG): c.10693 G>A in exon 39 of the ANK2 gene (NM_001148.4). Although rare, mutations in the ANK2 gene have been reported previously in association with LQTS (Mohler P et al., 2003). The E3565K variant has not been published as a mutation, nor has it been reported as a benign polymorphism to our knowledge. The E3565K variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The E3565K variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is moderately conserved among mammals. Additionally, in silico analysis predicts this variant is probably damaging to the protein structure/function. However, only a single missense mutation in a nearby residue (E3570K) has been reported in association with LQTS, indicating this region of the protein may be tolerant of change. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic mutation or a rare benign variant. The variant is found in ARRHYTHMIA panel(s).
