Likely pathogenic for Ehlers-Danlos syndrome, kyphoscoliotic type, 2 — the classification assigned by Broad Center for Mendelian Genomics, Broad Institute of MIT and Harvard to NM_017946.4(FKBP14):c.428C>G (p.Ser143Ter), citing ACMG Guidelines, 2015. This variant lies in the FKBP14 gene (transcript NM_017946.4) at coding-DNA position 428, where C is replaced by G; at the protein level this means converts the codon for serine at residue 143 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The homozygous p.Ser143Ter variant in FKBP14 was identified by our study in one individual with Ehlers-Danlos syndrome. The p.Ser143Ter variant in FKBP14 has not been previously reported in individuals with Ehlers-Danlos syndrome kyphoscoliotic type 2 but has been identified in 0.01% (4/31642) of Latino/Admixed American chromosomes by the Genome Aggregation Consortium (gnomAD, http://gnomad.broadinstitute.org; dbSNP ID:rs753355121). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. The affected individual identified by our study was a homozygote, which increases the probability that the p.Ser143Ter variant is pathogenic. This nonsense variant leads to a premature termination codon at position 143. This alteration occurs within the terminal 50 bases of the second to last exon and is more likely to escape nonsense mediated decay (NMD) and result in a truncated protein. Loss of function of the FKBP14 gene is an established disease mechanism in autosomal recessive Ehlers-Danlos syndrome kyphoscoliotic type 2. In summary, although additional studies are required to fully establish its clinical significance, this variant is likely pathogenic for autosomal recessive Ehlers-Danlos syndrome kyphoscoliotic type 2.
