This variant lies in the SMAD4 gene (transcript NM_005359.6) at coding-DNA position 1354 through coding-DNA position 1381, duplicating 28 bases; at the protein level this means shifts the reading frame starting at glutamine residue 461, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change inserts 28 nucleotides in exon 11 of the SMAD4 mRNA (c.1352_1354ins28), causing a frameshift at codon 461. This creates a premature translational stop signal (p.Gln461Argfs*42) and is expected to result in an absent or disrupted protein product.
