A presumed nomenclature of c.(?_-233)_(5193+1_5194-1)del has been designated for the purposes of this classification. Although exact breakpoints of this deletion are not known, it is expected to remove the initiation codon and is predicted to result either in absence of the protein or truncation of the encoded protein due to translation initiation at a downstream site. The variant was absent in 21694 control chromosomes in gnomAD database, structural variants data set. Deletion of exons 1-18 has been reported in the literature in multiple individuals affected with Hereditary Breast And Ovarian Cancer Syndrome (examples: Judkins_2012, Churpek_2015, Rebbeck_2018). These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. One submitter (CIMBA - an expert panel) provided clinical-significance assessments for this variant to ClinVar after 2014 and classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic.
