Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000059.4(BRCA2):c.5704G>T (p.Asp1902Tyr), citing Ambry Variant Classification Scheme 2023. This variant lies in the BRCA2 gene (transcript NM_000059.4) at coding-DNA position 5704, where G is replaced by T; at the protein level this means replaces aspartic acid at residue 1902 with tyrosine — a missense variant. Submitter rationale: The p.D1902Y variant (also known as c.5704G>T), located in coding exon 10 of the BRCA2 gene, results from a G to T substitution at nucleotide position 5704. The aspartic acid at codon 1902 is replaced by tyrosine, an amino acid with highly dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6502 samples (13004 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 225000 alleles tested) in our clinical cohort. This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of p.D1902Y remains unclear.
