NM_005359.6(SMAD4):c.1310T>G (p.Val437Gly) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome; Familial thoracic aortic aneurysm and aortic dissection by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the SMAD4 gene (transcript NM_005359.6) at coding-DNA position 1310, where T is replaced by G; at the protein level this means replaces valine at residue 437 with glycine — a missense variant. Submitter rationale: The p.V437G variant (also known as c.1310T>G), located in coding exon 10 of the SMAD4 gene, results from a T to G substitution at nucleotide position 1310. The valine at codon 437 is replaced by glycine, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.005% (greater than 20,000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. Using the BDGP and ESEfinder splice site prediction tools, this alteration is predicted to weaken the efficiency of the native acceptor splice site; however, direct evidence is unavailable. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.V437G remains unclear.
