NM_005359.6(SMAD4):c.1561A>C (p.Thr521Pro) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome; Familial thoracic aortic aneurysm and aortic dissection by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the SMAD4 gene (transcript NM_005359.6) at coding-DNA position 1561, where A is replaced by C; at the protein level this means replaces threonine at residue 521 with proline — a missense variant. Submitter rationale: The p.T521P variant (also known as c.1561A>C), located in coding exon 11 of the SMAD4 gene, results from an A to C substitution at nucleotide position 1561. The threonine at codon 521 is replaced by proline, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.005% (greater than 20000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.T521P remains unclear.
