Likely benign for Malignant tumor of breast — the classification assigned by Department of Pathology and Laboratory Medicine, Sinai Health System to NM_024675.4(PALB2):c.1095G>A (p.Arg365=): The PALB2 p.Arg365= variant was not identified in the literature nor was it identified in the Cosmic, MutDB, LOVD 3.0, or Zhejiang University databases. The variant was identified in dbSNP (ID: rs515726062) as "With Likely benign, Uncertain significance allele", and in ClinVar (classified as likely benign by Ambry Genetics and GeneDx). The variant was identified in control databases in 1 of 246168 chromosomes at a frequency of 0.000004 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the European population 1 of 111626 chromosomes (freq: 0.000009), but not in the African, Other, Latino, Ashkenazi Jewish, East Asian, Finnish, or South Asian populations. The p.Arg365 variant is not expected to have clinical significance because it does not result in a change of amino acid and is not located in a known consensus splice site. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) do not predict a greater than 10% difference in splicing. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time although we would lean towards a more benign role for this variant. This variant is classified as likely benign.
