Submitter rationale: The p.E991* pathogenic mutation (also known as c.2971G>T), located in coding exon 15 of the APC gene, results from a G to T substitution at nucleotide position 2971. This changes the amino acid from a glutamic acid to a stop codon within coding exon 15. This alteration occurs at the 3' terminus of theAPC gene, is not expected to trigger nonsense-mediated mRNA decay, and only impacts the last 1854 amino acids of the protein. However, premature stop codons are typically deleterious in nature and the impacted region is critical for protein function and a significant portion of the protein is affected (Ambry internal data). This alteration has been observed in at least one individual with a personal and family history that is consistent with classic familial adenomatous polyposis syndrome (Li H et al. Exp Ther Med, 2017 Apr;13:1495-1499). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation.
