Uncertain significance — the classification assigned by GeneDx to NM_000251.3(MSH2):c.476G>T (p.Arg159Ile), citing GeneDx Variant Classification (06012015). This variant lies in the MSH2 gene (transcript NM_000251.3) at coding-DNA position 476, where G is replaced by T; at the protein level this means replaces arginine at residue 159 with isoleucine — a missense variant. Submitter rationale: This variant is denoted MSH2 c.476G>T at the cDNA level, p.Arg159Ile (R159I) at the protein level, and results in the change of an Arginine to an Isoleucine (AGA>ATA). This variant has not, to our knowledge, been published in the literature as pathogenic or benign. MSH2 Arg159Ile was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, suggesting it is not a common benign variant in these populations. Since Arginine and Isoleucine differ in polarity, charge, size or other properties, this is considered a non-conservative amino acid substitution. MSH2 Arg159Ile occurs at a position that is conserved across species and is located in the connector domain (LÃ¼tzen 2008, Kansikas 2011). In silico analyses predict that this variant is probably damaging to protein structure and function. Based on currently available evidence, it is unclear whether MSH2 Arg159Ile is a pathogenic or benign variant. We consider it to be a variant of uncertain significance.
