Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000251.1(MSH2):c.-143A>T, citing Ambry Variant Classification Scheme 2023. This variant lies in the MSH2 gene (transcript NM_000251.1) at 143 bases upstream of the translation start (5' untranslated region), where A is replaced by T. Submitter rationale: The c.-143A>T variant located in the 5' untranslated region (5 UTR) of the MSH2 gene. This variant results from an A to T substitution 143 bases upstream from the first translated codon. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project.; however this position was not covered in the ESP. To date, this alteration has been detected with an allele frequency of approximately 0.004% (greater than 24,000 alleles tested) in our clinical cohort. Based on nucleotide sequence alignment, this position is well conserved in available vertebrate species. Since supporting evidence is limited at this time, the clinical significance of c.-143A>T remains unclear.
