Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_024675.4(PALB2):c.169T>A (p.Cys57Ser), citing Ambry Variant Classification Scheme 2023. This variant lies in the PALB2 gene (transcript NM_024675.4) at coding-DNA position 169, where T is replaced by A; at the protein level this means replaces cysteine at residue 57 with serine — a missense variant. Submitter rationale: The p.C57S variant (also known as c.169T>A), located in coding exon 3 of the PALB2 gene, results from a T to A substitution at nucleotide position 169. The cysteine at codon 57 is replaced by serine, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6497 samples (12994 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.006% (greater than 16000 alleles tested) in our clinical cohort (includes this individual). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be benign and tolerated by PolyPhen and SIFT in silico analyses, respectively. Since supporting evidence is limited at this time, the clinical significance of p.C57S remains unclear.
