NM_005359.6(SMAD4):c.776C>T (p.Thr259Ile) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome; Familial thoracic aortic aneurysm and aortic dissection by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the SMAD4 gene (transcript NM_005359.6) at coding-DNA position 776, where C is replaced by T; at the protein level this means replaces threonine at residue 259 with isoleucine — a missense variant. Submitter rationale: The p.T259I variant (also known as c.776C>T), located in coding exon 5 of the SMAD4 gene, results from a C to T substitution at nucleotide position 776. The threonine at codon 259 is replaced by isoleucine, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.01% (greater than 7500 alleles tested) in our clinical cohort (includes this individual). Based on protein sequence alignment, this amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of p.T259I remains unclear.
