Pathogenic for Distal myopathy with posterior leg and anterior hand involvement; Myofibrillar myopathy 5; Hypertrophic cardiomyopathy 26 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001458.5(FLNC):c.8130G>A (p.Trp2710Ter), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the FLNC gene (transcript NM_001458.5) at coding-DNA position 8130, where G is replaced by A; at the protein level this means converts the codon for tryptophan at residue 2710 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: This sequence change creates a premature translational stop signal (p.Trp2710*) in the FLNC gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 16 amino acid(s) of the FLNC protein. This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with filaminopathy (PMID: 15929027, 22961544). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 18314). Algorithms developed to predict the effect of variants on gene product structure and function are not available or were not evaluated for this variant. Experimental studies have shown that this premature translational stop signal affects FLNC function (PMID: 22961544, 26472074, 26969713). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. For these reasons, this variant has been classified as Pathogenic.
