NM_004655.4(AXIN2):c.1742G>A (p.Arg581His) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the AXIN2 gene (transcript NM_004655.4) at coding-DNA position 1742, where G is replaced by A; at the protein level this means replaces arginine at residue 581 with histidine — a missense variant. Submitter rationale: This variant is denoted AXIN2 c.1742G>A at the cDNA level, p.Arg581His (R581H) at the protein level, and results in the change of an Arginine to a Histidine (CGC>CAC). This variant has not, to our knowledge, been published in the literature as pathogenic or benign. AXIN2 Arg581His was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. Since Arginine and Histidine share similar properties, this is considered a conservative amino acid substitution. AXIN2 Arg581His occurs at a position that is conserved across species and is not located in a known functional domain (UniProt). In silico analyses are inconsistent regarding the effect this variant may have on protein structure and function. Based on currently available information, it is unclear whether AXIN2 Arg581His is pathogenic or benign. We consider it to be a variant of uncertain significance.
