NM_000051.4(ATM):c.811C>A (p.Leu271Ile) was classified as Likely benign for Malignant tumor of breast by Department of Pathology and Laboratory Medicine, Sinai Health System: The ATM p.Leu271Ile variant was not identified in the literature nor was it identified in the LOVD 3.0 database. The variant was identified in dbSNP (ID: rs730881339) as "With Uncertain significance allele" and ClinVar (classified as uncertain significance by Invitae, GeneDx, Ambry Genetics and Color). The variant was identified in control databases in 1 of 245770 chromosomes at a frequency of 0.000004 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the European population in 1 of 111466 chromosomes (freq: 0.000009), while the variant was not observed in the African, Other, Latino, Ashkenazi Jewish, East Asian, Finnish, or South Asian populations. This variant was identified by our laboratory in a patient with bilateral breast cancer and a co-occurring, pathogenic CHEK2 variant (c.349A>G, p.Arg117Gly), decreasing the likelihood that this ATM variant has clinical significance. The p.Leu271 residue is not conserved in mammals and 5 of 5 computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein; however, this information is not predictive enough to rule out pathogenicity. The variant occurs outside of the splicing consensus sequence and 4 of 4 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) do not predict a difference in splicing. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time although we would lean towards a more benign role for this variant. This variant is classified as likely benign.
