NM_000257.4(MYH7):c.2464A>T (p.Met822Leu) was classified as Uncertain significance for Hypertrophic cardiomyopathy by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the MYH7 gene (transcript NM_000257.4) at coding-DNA position 2464, where A is replaced by T; at the protein level this means replaces methionine at residue 822 with leucine — a missense variant. Submitter rationale: This sequence change replaces methionine, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 822 of the MYH7 protein (p.Met822Leu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with hypertrophic cardiomyopathy (PMID: 12820698, 19149795, 20975235, 25351510). ClinVar contains an entry for this variant (Variation ID: 181188). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on MYH7 protein function. This variant is found within a region of MYH7 between codons 181 and 937 that contains the majority of the myosin head domain. Missense variants in this region have been shown to be significantly overrepresented in individuals with hypertrophic cardiomyopathy (PMID: 27532257). This variant disrupts the p.Met822 amino acid residue in MYH7. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 12881443). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
