Likely pathogenic for Bartter syndrome — the classification assigned by Women's Health and Genetics/Laboratory Corporation of America, LabCorp to NM_000338.3(SLC12A1):c.421-1G>A, citing LabCorp Variant Classification Summary - May 2015. This variant lies in the SLC12A1 gene (transcript NM_000338.3) at the canonical splice acceptor site of the intron immediately before coding-DNA position 421, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: Variant summary: SLC12A1 c.421-1G>A is located in a canonical splice-site and is predicted to affect mRNA splicing resulting in a significantly altered protein due to either exon skipping, shortening, or inclusion of intronic material. Several computational tools predict a significant impact on normal splicing: three predict the variant abolishes a 3 prime acceptor site. However, these predictions have yet to be confirmed by functional studies. The variant was absent in 248066 control chromosomes. To our knowledge, no occurrence of c.421-1G>A in individuals affected with Bartter Syndrome, Type 1 and no experimental evidence demonstrating its impact on protein function have been reported. No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Based on the evidence outlined above, the variant was classified as likely pathogenic.
