Uncertain significance for Congenital multicore myopathy with external ophthalmoplegia — the classification assigned by 3billion to NM_000540.3(RYR1):c.14285C>T (p.Pro4762Leu), citing ACMG Guidelines, 2015. This variant lies in the RYR1 gene (transcript NM_000540.3) at coding-DNA position 14285, where C is replaced by T; at the protein level this means replaces proline at residue 4762 with leucine — a missense variant. Submitter rationale: The variant is observed at an extremely low frequency in the gnomAD v4.1.0 dataset (total allele frequency: <0.001%). Predicted Consequence/Location: Missense variant In silico tool predictions suggest damaging effect of the variant on gene or gene product [3Cnet: 0.99 (> 0.75, sensitivity 0.96 and precision 0.92)]. The same nucleotide change resulting in the same amino acid change has been previously reported to be associated with RYR1-related disorder (ClinVar ID: VCV001800878). However, the evidence of pathogenicity is insufficient at this time. Therefore, this variant is classified as VUS according to the recommendation of ACMG/AMP guideline.
