Likely pathogenic for Polycystic kidney disease, adult type — the classification assigned by Victorian Clinical Genetics Services, Murdoch Childrens Research Institute to NM_001009944.3(PKD1):c.11017-25A>G, citing ACMG Guidelines, 2015: Based on the classification scheme VCGS_Germline_v1.3.4, this variant is classified as Likely pathogenic. Following criteria are met: 0102 - Loss of function is a known mechanism of disease in this gene and is associated with polycystic kidney disease 1 (MIM#173900). (I) 0107 - This gene is associated with autosomal dominant disease. Polycystic kidney disease 1 (MIM#173900) is predominantly caused by monoallelic variants, with rare reports of biallelic variants causing disease (OMIM). (I) 0209 - Splice site variant proven to affect splicing of the transcript with uncertain effect on protein sequence. RT-PCR studies (Garvan Institute of Medical Research) have indicated this variant results in both skipping of exon 38 and retention of part of intron 37 due to activation of a cryptic splice site. These results are comparable to the c.11017-10C>A variant, which has been reported in multiple patients with ADPKD (PKD database, ClinVar, PMID: 17582161). Exon 38 skipping is also observed at lower levels in healthy controls. (SP) 0251 - This variant is heterozygous. (I) 0301 - Variant is absent from gnomAD (both v2 and v3). (SP) 0506 - Abnormal splicing is not predicted and nucleotide is poorly conserved. (SB) 0705 - No comparable variants have previous evidence for pathogenicity. (I) 0807 - This variant has no previous evidence of pathogenicity. (I) 0905 - No published segregation evidence has been identified for this variant. (I) 1007 - No published functional evidence has been identified for this variant. (I) 1208 - Inheritance information for this variant is not currently available in this individual. (I) Legend: (SP) - Supporting pathogenic, (I) - Information, (SB) - Supporting benign
