Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_005591.4(MRE11):c.657C>G (p.Asn219Lys), citing Ambry Variant Classification Scheme 2023. This variant lies in the MRE11 gene (transcript NM_005591.4) at coding-DNA position 657, where C is replaced by G; at the protein level this means replaces asparagine at residue 219 with lysine — a missense variant. Submitter rationale: The p.N219K variant (also known as c.657C>G), located in coding exon 6 of the MRE11A gene, results from a C to G substitution at nucleotide position 657. The asparagine at codon 219 is replaced by lysine, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6498 samples (12996 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.002% (greater than 65000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of p.N219K remains unclear.
