Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000264.5(PTCH1):c.2560+1G>T, citing Ambry Variant Classification Scheme 2023. This variant lies in the PTCH1 gene (transcript NM_000264.5) at the canonical splice donor site of the intron immediately after coding-DNA position 2560, where G is replaced by T; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.2560+1G>T intronic pathogenic mutation results from a G to T substitution one nucleotide after coding exon 15 of the PTCH1 gene. This alteration has been observed in at least one individual with a personal and/or family history that is consistent with PTCH1-related disease (Ambry internal data, Sun LS et al. J Dent Res, 2008 Jun;87:575-9). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. RNA studies have demonstrated that this alteration results in abnormal splicing in the set of samples tested (Ambry internal data, Sun LS et al. J Dent Res, 2008 Jun;87:575-9 ). In addition to the clinical data, alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation.
