Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_032043.3(BRIP1):c.2449T>C (p.Tyr817His), citing Ambry Variant Classification Scheme 2023. This variant lies in the BRIP1 gene (transcript NM_032043.3) at coding-DNA position 2449, where T is replaced by C; at the protein level this means replaces tyrosine at residue 817 with histidine — a missense variant. Submitter rationale: The p.Y817H variant (also known as c.2449T>C), located in coding exon 16 of the BRIP1 gene, results from a T to C substitution at nucleotide position 2449. The tyrosine at codon 817 is replaced by histidine, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 120000 alleles tested) in our clinical cohort. Based on protein sequence alignment, this amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.Y817H remains unclear.
