Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000251.1(MSH2):c.-239C>T, citing Ambry Variant Classification Scheme 2023. This variant lies in the MSH2 gene (transcript NM_000251.1) at 239 bases upstream of the translation start (5' untranslated region), where C is replaced by T. Submitter rationale: The c.-239C>T variant is located in the 5' untranslated region (5&rsquo; UTR) of the MSH2 gene. This variant results from a C to T substitution 239 bases upstream from the first translated codon. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. This position was not covered in the ESP. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 115000 alleles tested) in our clinical cohort. This nucleotide position is well conserved in available vertebrate species. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
