Uncertain significance for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_001267550.2(TTN):c.49949-1G>T, citing Ambry Variant Classification Scheme 2023: The c.22754-1G>T intronic variant results from a G to T substitution one nucleotide upstream from coding exon 93 of the TTN gene. Exon 93 is located in the A-band region of the N2-B isoform of the titin protein and is constitutively expressed in TTN transcripts (percent spliced in or PSI 100%). Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and will result in the creation or strengthening of a novel splice acceptor site. The resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNA decay; however, direct evidence is unavailable. The exact functional effect of the missing amino acids is unknown. This nucleotide position is highly conserved in available vertebrate species. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
