NM_022436.3(ABCG5):c.1068C>G (p.Phe356Leu) was classified as Uncertain significance for Sitosterolemia by Department of Cardiology, Mackay Memorial Hospital, citing ACMG Guidelines, 2015: This variant results in the substitution of phenylalanine by leucine at position 356. Based on ortholog sequence alignment of ABCG5 (sterolin-1), the affected residue is highly conserved across vertebrate species and is located in the intracellular loop/linker region between the nucleotide-binding domain and the first transmembrane domain. Phenylalanine is a large aromatic residue, and its replacement by leucine, although also hydrophobic, may alter local spatial packing required for transporter conformational changes during the sterol efflux cycle. The variant is rare in population databases and was identified in a patient with severe hypercholesterolemia and a clinically compatible hypercholesterolemia-related phenotype, supporting application of PM2_supporting. However, computational predictions were discordant, with SIFT predicting a deleterious effect and PolyPhen-2 predicting a benign effect; therefore, PP3 was not applied. In the absence of functional studies, definitive segregation data, and direct plasma phytosterol measurements, the available evidence was insufficient to establish either pathogenicity or benignity, and the variant was therefore classified as a variant of uncertain significance.
