Uncertain significance for Hereditary cancer-predisposing syndrome; Familial thoracic aortic aneurysm and aortic dissection — the classification assigned by Ambry Genetics to NM_005359.6(SMAD4):c.179C>A (p.Ala60Asp), citing Ambry Variant Classification Scheme 2023. This variant lies in the SMAD4 gene (transcript NM_005359.6) at coding-DNA position 179, where C is replaced by A; at the protein level this means replaces alanine at residue 60 with aspartic acid — a missense variant. Submitter rationale: The p.A60D variant (also known as c.179C>A), located in coding exon 1 of the SMAD4 gene, results from a C to A substitution at nucleotide position 179. The alanine at codon 60 is replaced by aspartic acid, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.005% (greater than 20000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be probably damaging and deleterious by PolyPhen and SIFT in silico analyses, respectively. Since supporting evidence is limited at this time, the clinical significance of p.A60D remains unclear.
