Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000038.6(APC):c.1627-2A>T, citing Ambry Variant Classification Scheme 2023. This variant lies in the APC gene (transcript NM_000038.6) at the canonical splice acceptor site of the intron immediately before coding-DNA position 1627, where A is replaced by T; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.1627-2A>T intronic pathogenic mutation results from an A to T substitution two nucleotides upstream from coding exon 13 in the APC gene. The resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; however, direct evidence is unavailable. The exact functional effect of the missing amino acids is unknown; however, other alterations that result in the loss of coding exon 13 have been detected in multiple patients with a personal and/or family history of colon polyposis (Ambry internal data). This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site. Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation.
