Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_003072.5(SMARCA4):c.1419+1G>T, citing Ambry Variant Classification Scheme 2023. This variant lies in the SMARCA4 gene (transcript NM_003072.5) at the canonical splice donor site of the intron immediately after coding-DNA position 1419, where G is replaced by T; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.1419+1G>T intronic variant results from a G to T substitution one nucleotide after coding exon 7 of the SMARCA4 gene. This nucleotide position is highly conserved in available vertebrate species. Alterations that disrupt the canonical splice site are expected to result in aberrant splicing. In silico splice site analysis predicts that this alteration will weaken the native splice donor site and will result in the creation or strengthening of a novel splice donor site. RNA studies have demonstrated that this alteration results in a transcript predicted to lead to a protein with an in-frame deletion of 58 amino acids; however, the exact functional impact of the deleted amino acids is unknown at this time (Ambry internal data). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
