Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000251.3(MSH2):c.1289A>G (p.Lys430Arg), citing Ambry Variant Classification Scheme 2023. This variant lies in the MSH2 gene (transcript NM_000251.3) at coding-DNA position 1289, where A is replaced by G; at the protein level this means replaces lysine at residue 430 with arginine — a missense variant. Submitter rationale: The p.K430R variant (also known as c.1289A>G), located in coding exon 8 of the MSH2 gene, results from an A to G substitution at nucleotide position 1289. The lysine at codon 430 is replaced by arginine, an amino acid with highly similar properties. This amino acid position is poorly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. In addition, this alteration is predicted to be benign by MAPP-MMR in silico analyses (Chao EC et al. Hum. Mutat. 2008 Jun;29:852-60). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
