Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000314.4(PTEN):c.-960C>T, citing Ambry Variant Classification Scheme 2023. This variant lies in the PTEN gene (transcript NM_000314.4) at 960 bases upstream of the translation start (5' untranslated region), where C is replaced by T. Submitter rationale: The c.-960C>T variant located in the 5' untranslated region (5&rsquo; UTR) of the PTEN gene. This variant results from a C to T substitution 960 bases upstream from the first translated codon. This variant is located in the promoter region of the PTEN gene, but its potential impact on PTEN regulation has not yet been investigated (Zhou XP et al. Am J Hum Genet. 2003;73:404-411). This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project.; however this position was not covered in the ESP. To date, this alteration has been detected with an allele frequency of approximately 0.002% (greater than 51000 alleles tested) in our clinical cohort. This nucleotide position is poorly conserved in available vertebrate species. Since supporting evidence is limited at this time, the clinical significance of c.-960C>T remains unclear.
