NM_001089.3(ABCA3):c.873G>A (p.Lys291=) was classified as Uncertain significance for Hereditary pulmonary alveolar proteinosis by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The c.873G>A variant (also known as p.K291K) located in coding exon 5 of the ABCA3 gene. This variant results from a G to A substitution at nucleotide position 873. This nucleotide substitution does not change the at codon 291. However, this change occurs in the last base pair of coding exon 5 which makes it likely to have some effect on normal mRNA splicing. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6498 samples (12996 alleles) with coverage at this position. This nucleotide position is highly conserved in available vertebrate species. Using the BDGP and ESEfinder splice site prediction tools, this alteration is predicted to weaken the efficiency of the native donor splice site; however, direct evidence is unavailable. Since supporting evidence is limited at this time, the clinical significance of this variant remains unclear.
